The expansion represents an important step in CareOne Senior Care’s continued commitment to individuals living with Alzheimer’s disease and other forms of dementia, as well as to the family members who care for them every day.
What Is the GUIDE Program?
The GUIDE Model — Guiding an Improved Dementia Experience — is a nationwide Medicare initiative developed by the Centers for Medicare & Medicaid Services (CMS) to improve the quality of life for people living with dementia and to provide meaningful support to their unpaid family caregivers.
For many families, caring for a loved one with dementia can become overwhelming. Families are often left trying to coordinate medical care, understand changes in their loved one’s condition, locate community resources, and provide increasing amounts of supervision and personal care, often without knowing what assistance is available.
GUIDE was created to change that.
By participating in GUIDE dementia care programs, eligible individuals and their caregivers may receive a comprehensive package of dementia-related services and support with no patient cost-sharing for GUIDE services.
Depending on eligibility and individual needs, GUIDE services may include:
- A dedicated care navigator to help families coordinate dementia care and access resources.
- A comprehensive assessment and individualized care plan.
- Caregiver education, skills training, and support.
- Access to a 24/7 support line for questions and concerns.
- Coordination between healthcare providers and community resources.
- Assistance identifying services such as transportation, meals, and other community supports.
- Up to $2,625 per year in respite services for eligible individuals with a qualifying unpaid caregiver.
- In-home respite care that allows family caregivers time to rest, attend appointments, work, travel, spend time with other family members, or simply take a much-needed break.
Who May Qualify for GUIDE?
The GUIDE Model is specifically designed for people living with dementia, including Alzheimer’s disease and other forms of dementia.
Generally, an individual must:
- Have dementia confirmed by a clinician participating in a GUIDE dementia care program.
- Be enrolled in Original Medicare Parts A and B.
- Have Medicare as their primary payer.
- Not be enrolled in Medicare Advantage or PACE.
- Not have elected the Medicare hospice benefit.
- Not be a long-term nursing home resident.
- Not already be aligned with another GUIDE participant.
Individuals may be eligible at mild, moderate, or severe stages of dementia. A family does not have to wait until dementia becomes advanced to explore GUIDE.
Respite Care: Caring for the Caregiver, Too
One of the most significant benefits of GUIDE is its recognition that dementia affects the entire family.
Spouses, adult children, grandchildren, and other family members frequently become unpaid caregivers. Over time, caregiving responsibilities can affect their careers, families, physical health, and emotional well-being.
GUIDE recognizes that supporting the caregiver is an essential part of supporting the person living with dementia.
For qualifying families, respite services can provide temporary relief from caregiving responsibilities.
