This expansion represents an important step in our continued commitment to individuals living with Alzheimer’s disease and other forms of dementia, and to the family members who care for them every day.
Understanding Medicare’s GUIDE Model
What the model covers, and why it was created
The GUIDE Model — Guiding an Improved Dementia Experience — is a nationwide Medicare initiative developed by the Centers for Medicare & Medicaid Services (CMS) to improve the quality of life for people living with dementia and to provide meaningful support to their unpaid family caregivers.
For many Farmington Hills families, caring for a loved one with dementia can become overwhelming. Families are often left trying to coordinate medical care, understand changes in their loved one’s condition, locate community resources, and provide increasing amounts of supervision and personal care, often without knowing what assistance is available. GUIDE was created to change that.
Through participating in GUIDE dementia care programs, eligible individuals and their caregivers may receive a comprehensive package of dementia-related services and support with no patient cost-sharing for GUIDE services. Depending on eligibility and individual needs, GUIDE services may include:
Care navigation: a dedicated care navigator to help families coordinate dementia care and access resources.
Assessment and planning: a comprehensive assessment and individualized care plan.
Caregiver education: skills training and ongoing support for family caregivers.
24/7 support line: access to answers whenever questions or concerns come up.
Coordination: connecting healthcare providers with community resources.
Community supports: help identifying services such as transportation and meals.
In-home respite care: time for family caregivers to rest, attend appointments, work, travel, or take a break.
GUIDE Eligibility: What Farmington Hills Families Should Know
Who the program is designed to serve
The GUIDE Model is designed specifically for people living with dementia, including Alzheimer’s disease and other forms of dementia. Generally, an individual must:
Have dementia confirmed by a clinician participating in a GUIDE dementia care program.
Be enrolled in Original Medicare Parts A and B.
Have Medicare as their primary payer.
Not be enrolled in Medicare Advantage or PACE.
Not have elected the Medicare hospice benefit.
Not be a long-term nursing home resident.
Not already be aligned with another GUIDE participant.
Individuals may be eligible at mild, moderate, or severe stages of dementia. A family doesn’t have to wait until dementia becomes advanced to explore GUIDE.
